Engagement of Substance-Using Pregnant Women in Addiction Recovery

This article reports on a qualitative study examining the experiences and recovery needs of substanceusing pregnant women, with a primary focus on women's engagement by child protection services (CPS) in addiction recovery programs. Current CPS policies and practices have largely failed to engage these women, despite women's stated desire for recovery support during pregnancy. Three core themes related to women's primary need for safety—the need for collaborative relationships with CPS, children as motivators for change, and needed modifications in the social environment—are discussed. Implications for social policy and direct practice in regard to CPS engagement of substance-using pregnant women in recovery programs are examined.

Cette recherche vise à mieux comprendre le rôle de l'hébergement dans le rétablissement des personnes vivant avec une maladie mentale grave. Quarante résidents et résidentes de différents types d'hébergement communautaire structuré ont été interrogés sur les conditions d'hébergement qui favorisent leur rétablissement. Les résultats indiquent qu'aux yeux des résidents et résidentes, les conditions matérielles d'existence ont une influence déterminante sur le rétablissement. Leurs préoccupations s'articulent autour des besoins de base tels que l'alimentation, le coût, les commodités et la qualité de leur hébergement. Les relations sociales avec leur entourage immédiat ont également une grande influence sur le rétablissement des personnes, lesquelles souhaitent d'abord conserver leurs modestes acquis, tant matériels que sociaux. Cette stabilité est prérequise à leur rétablissement.

“Stuck in the Mud”: Limited Employment Success of Persons With Serious Mental Illness in Northeastern Ontario

Despite policy support, empirical evidence, investment in community mental health programs, and a good deal of rhetoric about promoting recovery, people with serious mental illness (SMI) remain disproportionately represented in paid employment, especially in northern and rural places in Ontario. This study examines access to employment through the perspectives of people with SMI, providers, and decision makers who reside in two northeastern Ontario case communities. A qualitative case study using community-based participatory research methods was employed. Data from interviews conducted with 46 participants were analyzed thematically and were complemented by a secondary data source reporting on the employment outcomes of 4,112 people with SMI. This paper reports on the qualitative findings, highlighting how employment for persons with SMI is stuck in the mud by a dominant community discourse conveying a disbelief in the capacity of people with SMI to be employed—a discourse sustained by a variety of local and systemic tensions in local practices. This study underscores the ineffectiveness of current employment programs for people who experience SMI in the case communities, and the need to develop local capacity to provide evidence-based practices to improve employment success, and subsequently, to shift marginalizing discourses.

Challenges in Implementing Recovery-Based Mental Health Care Practices in Psychiatric Tertiary Care

Despite increased interest in the concept of recovery, not enough is known about the challenges of implementing recovery models in mental health care settings. Findings are presented from a 3-year feminist ethnographic study that followed recently deinstitutionalized women and men as they moved into psychiatric tertiary care facilities in British Columbia where a psychosocial rehabilitation model based on recovery principles was implemented. We found that inconsistent staff training and stretched community supports have resulted in uneven implementation that does not yet maximize opportunities for people's recovery. Further, care is organized and delivered in ways that emphasize individual needs as opposed to social and collective needs based on factors such as gender, ethnicity, and culture. These findings indicate that greater political will, as measured in commitments to community-based mental health services, is required to fully realize the philosophy of recovery and equitable mental health care.

No More “Us” and “Them”: Integrating Recovery and Well-Being into a Conceptual Model for Mental Health Policy

To set the stage for this special edition on Responses to the Mental Health Strategy for Canada: Canadian and International Perspectives on Mobilizing Change, we discuss the role of ideas in the public policy literature and the influence of key ideas over the history of mental health policy. Drawing on academic and policy literature and feedback from a convenience sample of mental health policy makers, we integrate the concepts of recovery and well-being into a conceptual model that can be used by policy makers as a tool to realize the transformative ideas captured in the Mental Health Strategy for Canada.

What Are the Personal Recovery Needs of Community-Dwelling Individuals with Mental Illness? Preliminary Findings from the Canadian Personal Recovery Outcome Measurement (C-PROM) Study

Personal recovery is an overarching goal that underpins mental health community support and clinical services in Canada. Surprisingly, little information is available about the personal recovery needs of community-dwelling Canadians with mental illness. The purpose of this study is to describe these needs in a sample of adults living in a large urban centre. We performed a multi-site cross-sectional survey of adults who receive community-based mental health services in an urban setting. Participants were asked to complete six patient-reported outcome (PRO) measures about personal recovery, hope, mastery, and depression. We used descriptive and correlational analyses to summarize the data according to an established evidence-based theoretical conceptualization of recovery. Two hundred and twenty-eight individuals participated in the study. Participants indicated high levels of self-reported empowerment and hope and optimism about the future. In contrast, participants reported low levels of perceived connectedness, identity, and meaning in life. In conclusion, this study identified that recovery-oriented PRO measures can improve our understanding of the needs and goals of individuals with mental illness and highlight the ways in which individuals can achieve a sense of meaning in life. Understanding the recovery needs of Canadians with mental illness may help foster recovery-oriented healthcare by bringing person-centred approaches to the point of treatment, so as to ensure greater quality and accountability of mental health services.

The Implementation of a Rent Assistance Program and Its Impacts on Recovery Outcomes for Individuals Experiencing Chronic Homelessness

This study explored the experiences of persons experiencing chronic homelessness with and without rent assistance. Qualitative interviews were conducted with 12 participants in order to determine if and how rent assistance impacted participants’ narratives of recovery. These narrative interviews, along with a focus group conducted with 10 support workers, explored implementation factors impacting recovery narratives. The findings suggest that access to rent assistance helps to promote recovery outcomes, conceptualized as life transitions (streets to home, home to community, and past to future), among individuals experiencing chronic homelessness. These transitions were enhanced by various participant, program, and community factors.

Étude sur l’expérience des relations amoureuses des jeunes hommes ayant vécu un premier épisode psychotique

Cette étude qualitative s’intéresse aux facteurs pouvant expliquer les difficultés rencontrées par les jeunes hommes (18–35 ans) ayant vécu un premier épisode psychotique à établir et maintenir des relations amoureuses. Douze participants ont pris part à une entrevue semi-structurée d’environ 60 minutes. Quatre thèmes principaux sont ressortis de l’analyse des données, soit la Gestion de la proximité, la Conception de l’intimité, l’Identité et le Processus de rétablissement. Les difficultés rencontrées seraient liées à des expériences relationnelles antérieures et à des difficultés métacognitives et de régulation émotionnelle suite à un épisode psychotique, mais des études supplémentaires sont requises.

A Roof for Recovery: Combining the Principles of Housing First and Early Intervention in Services for People with Psychosis

The availability of affordable, youth-friendly, recovery-oriented housing for persons with first-episode psychosis is limited. We report on the conceptualization, implementation and implications of a pilot housing project combining the principles of specialized early intervention for psychosis and those of “Housing First,” which holds much promise to address this service gap.

Impact des facteurs structurels et des processus de travail des équipes sur les personnes utilisatrices de trois réseaux locaux de services en santé mentale dans un contexte de réforme

Cet article explore l’impact des structures et processus de travail des équipes sur les personnes utilisatrices dans trois réseaux de services en santé mentale du Québec dans un contexte de réforme. Des coordonnateurs (N = 41), professionnels de soins (N = 311) et des personnes utilisatrices (N = 327) ont été sondés. Les effets sur les personnes utilisatrices dans le troisième réseau, semi-urbain, sont similaires à ceux du premier réseau, urbain et mieux nanti. L’autonomie d’équipe, la participation décisionnelle et des interactions avec les partenaires du réseau peuvent contribuer à améliorer la continuité des services, la qualité de vie et le rétablissement des personnes utilisatrices malgré un financement moindre.

Recovery is a policy framework for mental health in Canada. Key challenges to the integration of recovery include a gap in knowledge about the work that people do to promote their health and well-being in the context of living with mental ill health. This study used Photovoice to explore the lived realities of people living with mental ill health and the impact of the social determinants on their recovery process. Findings from this study inform policy and practice on promoting health work as an important dimension of recovery and community inclusion.

Accessing and Utilizing One’s City Space: The Role of Specialized Community Mental Health Teams in Brazil and Canada

Community mental health programs have garnered significant attention during the last decade. In this paper we ask to what extent these programs impact the capacity of service users to move around in the city space. Drawing on case studies from ethnographic research conducted in Campinas (Brazil) and Montréal (Canada), which included semi-structured interviews with a total of 16 service users and 49 mental health professionals, we explore the significance of urban mobility as part of service users’ mental health recovery and service providers’ practice. Findings suggest that service providers play a key role in facilitating meaningful mobility in the city space.

Beyond the Barriers: First-Voice Perspectives on Facilitators of Leisure Participation

Participation in community-based leisure activity can be beneficial for supporting recovery for people with mental illness, but many barriers are faced. Drawing on first-voice perspectives, this study examined personal, social, and organizational factors that facilitate community-based leisure participation and suggests practical ways to create leisure environments that they would find supportive and more accessible. Five focus groups were conducted with adults in urban and rural communities in Nova Scotia. Through thematic analysis, five types of facilitators were identified: self-management and self-care; finding meaning; active social supports and connections; welcoming environments; and, educating others. Implications for practice are presented.

A Values-Based Analysis of Recovery-Oriented Practice in Mental Health Care and Medical Assistance in Dying

The Criminal Code of Canada has been amended to allow medical assistance in dying (MAiD) under prescribed criteria. There has been considerable debate regarding whether people with mental illness as the sole underlying medical condition should be eligible. It is argued that access to MAiD is not compatible with recovery-oriented care. Based on a comprehensive analysis exploring the ethical principles guiding decision making around MAiD, this paper offers a discussion of the compatibility between MAiD and recovery-oriented care and demonstrates significant overlap of these principles. The discussion around MAiD as an option in recovery-oriented care is legitimate and needs to continue.

Transitional employment (TE) within the Clubhouse model has been adopted across Canada and the world for its effectiveness in promoting work and recovery for persons living with mental illness. However, attempts to establish a TE program in the province of Québec have been met with numerous roadblocks. Using a qualitative phenomenological approach, this study aimed to synthesize knowledge that could inform local implementation of TE. Experiences of Montréal Clubhouse members and Canadian Clubhouse directors were examined. Member narratives identified employment-program and systemic shortcomings, while directors provided key strategies for overcoming barriers to TE implementation.

An Accessible Mental Health Self-Management Innovation for College Students

This article outlines findings from two Wellness Recovery Action Plan® workshops for college students in Western Canada. Widely recognized mental health challenges among college students and a service need demonstrated in pilot sessions served as rationale for providing one of the first iterations of WRAP in a college setting.

Guérison autochtone et rétablissement en santé mentale : rapprochement conceptuel et implications pour l’intervention psychosociale

La guérison autochtone fait présentement l’objet d’un intérêt inégalé dans le champ de la santé mentale. En parallèle, les services réguliers en santé mentale au Québec et au Canada restent marqués par le paradigme du rétablissement. Si ces approches partagent plusieurs traits communs, elles présentent aussi des particularités et des nuances importantes à maîtriser pour les intervenant.es. Le présent article propose ainsi une comparaison conceptuelle en profondeur de ces deux approches. Quatre dimensions ont permis de les rapprocher et de les contraster, soit : 1) la pluralité de leurs assises conceptuelles ; 2) la globalité de leur vision des problématiques et de l’intervention ; 3) la non-linéarité des cheminements personnels et collectifs ; 4) la non-directivité des processus de changement. En discussion, nous abordons les implications pratiques de l’analyse conceptuelle. En conclusion, l’article soutien une hybridation possible, voire souhaitable entre les approches autochtones et occidentales dans le domaine de la santé mentale.
